To work closely with members of the multi-professional team within St Margaret of Scotland Hospice and the wider community projects/services to provide specialist palliative care within the Hospice and community setting. To participate in daily Ward Rounds, weekly Grand Ward Rounds and daily Patient handovers within the Specialist Palliative Care Centre – St Joseph’s Ward. To establish oneself as a resource, facilitator and role model for Specialist Palliative Care within the Hospice in-patient and community settings. To adopt a multi-professional and collaborative team approach to care assessment and delivery to ensure the needs of community-based patients and families are met. To participate in the Hospice Virtual Clinic and/or deputise for the Community Palliative Care Lead as and when required. To liaise with the Hospice Social Worker to optimise the psychosocial aspects of the patient and family from a quality-of-life perspective. To liaise with the Hospice Therapy Team to optimise the functional aspects of the patient and family from a quality-of-life perspective. To support families/carers in bereavement, referring appropriately to child/young person and adult Bereavement Services, should long-term support be required.
