Support the lead pharmacist for emergency care in the development of clinical pharmacy services. Providing core clinical pharmacy service to emergency medicine in line with the Trust and clinical pharmacy standards. Ensuring high quality prescribing and care is provided on and during admission through to discharge to patients by effective clinical screening. Ensuring that pharmaceutical knowledge and skills are applied through medication reconciliation on admission, optimisation during their admission and input to planning for a successful discharge process where appropriate. Identify medicine-related causes for admission and develop treatment plans to optimise care. Actively providing medicines information and medication counselling to health care professionals, patients and/or carers. Working with medical staff to produce medication policies and guidelines that promote rationale & economical prescribing with appropriate monitoring practice to ensure policies and guidelines are adhered to. Assist in the development and implementation of systems for prescribing management within the Trust, to ensure the safe, economic and clinically effective use of medications in all patients in line with local and national initiatives. Working with relevant staff to reduce risks in medicine prescribing, storage & administration. Ensuring legal requirements are met. Reporting of clinical incidents and responding to drug errors by helping to review practice and decrease risks.
