The post is to increase access to pulmonary rehabilitation. This will involve working collaboratively with referrers and the integrated care board to highlight health inequalities and offer other venues for pulmonary rehabilitation across Calderdale.
There will be an opportunity to be involved in the post-COVID service experiencing the self-management groups delivered jointly by a physiotherapist and an occupational therapist.
Main duties of the job To provide specialist assessment of patients with chronic respiratory disease including exercise testing and provide appropriate treatment to individuals in a variety of settings. To work within the community respiratory multidisciplinary team to promote self-management and hospital avoidance in patients with chronic respiratory disease. To provide ambulatory oxygen assessments to both chronic respiratory and long COVID patients. To be the lead of a community-based pulmonary rehabilitation group. To work in conjunction with the clinical lead to scope lack of provision of pulmonary rehabilitation to certain groups of people. To improve access to pulmonary rehabilitation which may involve looking at community buildings, liaising with stakeholders and holding focus groups looking at barriers to rehabilitation. Working with other key partners to evaluate services offered to specific groups of people such as people with learning disabilities or non-English speaking patients. To provide clinical outcomes for all patients enrolled at an additional venue.
Job responsibilities Act as a specialist physiotherapist within the pulmonary rehabilitation team and contribute to evolving and embedding best practice into the clinical area. Manage autonomously specialist physiotherapy caseload in chronic respiratory disease. Carry out specialist assessment and rehabilitation of patients with respiratory disease, including implementing, evaluating and modifying individual treatment programmes. Possess a working knowledge of the community oxygen assessment service and the ability to complete an ambulatory oxygen assessment if required. Develop effective links with other community and specialist respiratory services including General Practitioners, Practice Nurses and Respiratory Physicians, mental health services, and Community Pharmacists to provide specialist advice where needed and refer to other agencies as required. Take part in the education and training of Physiotherapists and qualified and non-qualified healthcare/social services/education staff including medical staff. Support the Clinical Lead in the day-to-day running of the pulmonary rehabilitation team including support staff. At times when required by the clinical lead, to manage patients at home with an exacerbation of COPD who would previously have needed hospital in-patient management. Implement strategies and initiate a treatment plan for managing patients with complex respiratory needs in the community settings. To work within the community respiratory service, developing effective clinical interventions within the speciality of respiratory in order to contribute to clinical practice in line with the current evidence base.
Person Specification QUALIFICATIONS / TRAINING Essential Physiotherapy Degree Registered with HCPC Clinical Educator training Computer literate with experience in using health-related systems such as SystmOne, EPR and EMIS
Desirable Evidence of postgraduate training in the respiratory field Member of the Professional body Member of a respiratory network/ACPRC Experience in respiratory care/pulmonary rehabilitation/group work
KNOWLEDGE, EXPERIENCE & EXPERTISE Essential Recent experience demonstrating ability to assess, treat and evaluate care within the core area of both respiratory physiotherapy and long COVID within the NHS Excellent clinical reasoning skills and the ability to maintain accurate records Previous experience in Pulmonary rehabilitation and/or respiratory care including planning and delivering programs, and delivering group education sessions Evidence of recent experience in supervising, developing and training others The ability to lead assessment and treatment sessions and create detailed and comprehensive treatment plans for complex respiratory patients To have an understanding of the pathophysiology of chronic lung disease applying this to a detailed patient assessment and the further prescription of an exercise plan Previous practical experience of performing or observing oxygen assessment and prescription for ambulatory and/or long-term therapy
Desirable Experience of group work in particular presenting to patients and other professionals Relevant experience in contributing to service improvements e.g. Audit, patient information etc.
