This post is to support delivery of the Integrated Respiratory Service within Hillingdon borough. The appointee will join the existing multi-disciplinary respiratory team and work in conjunction with the team members in coordinating and developing the service. The post holder will work collaboratively with healthcare professionals and stakeholders involved in the provision of integrated care, to ensure joined up care for patients across acute, community, primary and secondary care settings. This post would suit a Nurse looking to develop their respiratory knowledge base and gain experience in delivering Integrated Respiratory Care. The post holder will develop a good working knowledge in the diagnosis and management of Asthma and COPD, hone their clinical assessment skills, and experience working as an autonomous practitioner. Main duties of the job

To plan, evaluate treatment and care for patients using the required assessment tools, agreed pathways and clinical guidelines. To assess patients on wards/departments for suitability for Early Supported Discharge. To provide clinical assessment in patient's homes, making management decisions with the support of Band 7/Integrated Respiratory Consultant when appropriate. To participate in Integrated Care MDTs and case conferences. To support admission avoidance schemes currently operating within the department, whereby assessment of acutely unwell patients in a variety of settings including patient homes, General Practice, community sites and ambulatory/secondary care clinics is required. To provide support for the COPD HOT line for both patients and General Practitioners to provide knowledge/information and to advise on exacerbation management and suitability for admission avoidance/home assessment. To support the Covid Virtual Ward, providing virtual assessments and monitoring. To participate in audit and service development projects to assess service delivery and ensure best practice. To provide education and expert resource to practice nurses, the wider community multidisciplinary team, and general practitioners. Person Specification

Diploma or degree in Nursing. Current NMC registration. Evidence of CPD. Full UK driving license and access to a car. Knowledge and experience in the same or related field. Experience in managing respiratory patients. Non Medical Prescribing. ARTP Certification. Experience of community working. Audit and research experience. Disclosure and Barring Service Check
