Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Specialist Social Worker within the Adult Mental Health & Learning Disability Team to work Full Time based in Hull. The salary for this Specialist Social Worker job is up to £41,511 per annum. Main duties: Conduct skills active listening and confidently facilitate difficult conversations to ensure people’s hopes and wishes are efficiently and timely planned and supported. Support the development of staff/team to promote an approach to people who use services that is based on the principles of co-production. Supervises and develops staff through a variety of forum e.g. 1-1 practice development, reflected practice, mentor, coaching including providing honest and constructive challenge/feedback. Engages in complex cases that require autonomous decision-making, multi-agency/disciplinary co-ordination and or legal preparation and planning. Coordinate services and people in relation to a transition of care and support for young people moving into adult services. To support families and carers to adjust to changing circumstances. Contributes to team planning, effectiveness and responsiveness. Supports team manager with improving practice and performance of individuals/team by providing day-to-day education and supervision. Actively engages in practice development forums/meetings ensuring focussed and purposeful discussions lead to improved social work practice providing professional input as required. Contribute to the education/learning and development of newly qualified/newly appointed social workers by overseeing their induction/completion of ASYE. As directed prepares and attends decision making forums including supporting co-workers with decisions relating to complex/high risk cases. Works collaboratively with colleagues to ensure that people’s outcomes are achieved e.g. facilitating joint reviews/assessments. Ensures that case recording is accurate, up to date, in line with legislation and is available for a variety of purposes e.g. Multi-agency, meetings, strategy meetings, panels, reviews, court. Supports Line Manager to review working practices to deliver a continuously improving responsive service that recognises individual need, strengths and resilience communicates and encourages innovation, motivation, understanding and vision.
