To be a Specialist Speech and Language Therapist within the Community Learning Disability Team in Bolton. This Team provides a multi-disciplinary / agency service to adults with a learning disability, in respect of both health and social care. To be a core member of the Community Learning Disability Team, taking a full part in all multidisciplinary working including training, decision making, case conferences and scheduled meetings, ensuring well-coordinated care. To provide a specialist diagnostic and therapeutic service to adults with a learning disability across Bolton who have communication difficulties and/or dysphagia and who are undergoing active management by the team. Patients will be from a variety of ethnic and social backgrounds. These patients may also present with challenging behaviour and/or mental health issues. To liaise with, train, and work in partnership with patients, relatives, paid and unpaid carers and multi-agency workers and professionals to ensure the delivery of a high-quality service.
