Main duties of the job
To ensure that an individual programme of care is planned, implemented and evaluated for each patient within own sphere of responsibility. To work as part of a multi-disciplinary team ensuring the delivery of a high standard of care to the patients. Ensure all patients receive a high standard of care in line with evidence-based practice. Ensure adherence to Trust/Local policies, guidelines and other legislation and support National and Local initiatives, adhering to the NMC Code of Professional Conduct at all times. Take part in rotation through all areas of the departments and across the two sites to meet the needs of the service which currently covers the hours between 7:00 am and 10:00 pm.

Essential
1st & 2nd level registration (RGN) Adult Branch Nursing training as appropriate to the post.
Desirable
997/998/MIPS/SSSA. Advance Medical Prescribing.
Knowledge
Essential
Knowledge of clinical information and management.
Desirable
Experience of working in a multi-disciplinary environment.
Communication Skills
Essential
Effective communication with all members of the multi-disciplinary team. Basic IT Skills.
Desirable
Experience of Medway & Database management. A working knowledge of EPR, ACP, Infoflex, and ICE.
