The Wolds are seeking a band 5 Registered Mental Health Nurse (RMHN) who is committed to providing high-quality care. We are offering a permanent role to work alongside our current nursing team and be part of the multidisciplinary team that really delivers. Main duties of the job

As a staff nurse on the Wolds, you will be allocated a caseload of named service users, as well as associate service users. There is also the expectation that you remain up to date with the care and treatment for all admitted onto the Wolds. As a named nurse, you will be responsible for the duties this role entails, such as personalized care plans that are tailored to the individual, written with them and not for them, and relevant paperwork that helps aid recovery. You will also have the opportunity to facilitate groups specific to our clients' needs. Medicine management, participation in MDTs, working closely with our Social Inclusion Workers for the best outcome for our patients through their reablement pathway. As the Wolds is a 24/7 service, we have to ensure safe staffing levels at all times. The working shifts within the rehabilitation service are long days with a rotation of nights. Perform nursing assessments of a client’s personal needs using relevant skills and assessment tools. Implement a range of clinical skills in the assessment of client risk. Construct client care plans and risk management plans based on their assessed needs. Perform nursing interventions advocated within the clients' care plan. Recognise the contribution of others in the provision of client care. Involve clients and/or carers in the care planning and care delivery in a compassionate and appropriate manner which considers their diverse needs. Maintain the welfare and safety of clients and protect them from any form of abuse in accordance with safeguarding guidance. Monitor the impact and effectiveness of nursing care delivered and revise care plans as appropriate. Demonstrate knowledge of information governance and the need to protect patient confidential information. Make and maintain accurate records of nursing care in accordance with Professional (NMC) guidance and Trust policy. Demonstrate problem-solving skills by identifying solutions in routine clinical issues. Participate in regular clinical supervision. Qualifications

RMN or RNLD 1st Level Registration (NMC) or degree or equivalent Evidence of specialized continued professional training in clinical practice. Experience

Demonstrating supportive and sensitive communication to patients, carers, and staff, whilst demonstrating an understanding of the nature and effects of some information and barriers to effective communication while being considerate of client confidentiality Relevant experience of working with mental health needs Sound knowledge of the national agenda for mental health Sound knowledge of clinical/risk assessment and understanding of Information Governance principles. Previous experience working as a registered mental health nurse with people with mental health problems.
