A wonderful opportunity has arisen for an enthusiastic staff nurse to join our friendly, caring, well-established School Nursing Targeted Intervention team. The post is for 30 hours per week whole year round. Our team covers the Stoke on Trent area and is responsible for supporting children/young people (CYP) with complex, physical, or emotional health needs. Our aim is to provide support to CYP, their families, and the Schools so they can attend School safely and achieve to the very best of their ability.
This position will entail predominantly supporting our Special Schools and CYP with care planning, teacher training, enuresis clinics, continence support, and reviewing health assessments for our children in care.

Main duties of the job
The School Nursing Targeted Intervention Service prides itself on providing health advice and support to children, young people, and their families within Stoke on Trent.
The key responsibilities of the role include: working closely with our special and mainstream schools in the Stoke on Trent area, supporting teacher training and care planning for children with specific health needs, delivering nocturnal enuresis clinics, toileting/continence issues, advice and support to parents regarding the physical and emotional health needs of their child, and completing review health assessments with looked after children. The post holder will need to have excellent communication skills and be confident in the roles of safeguarding children.
The post is a permanent post for 30 hours per week, the service is delivered between Monday to Friday, 9.00 am to 5.00 pm. There will be flexibility for the hours to be delivered within these days and times.

2. Job Purpose/Main Duties/Responsibilities
The post holder will be responsible for their own caseload within their specialty area as an autonomous practitioner. In doing so, they will assess, plan, deliver, and evaluate patient-centered programs of care to a specialist level ensuring that they practice within their sphere of competence and knowledge. The School Health Service is integral to the delivery of the Healthy Child Programme; it is the universal prevention and early intervention service for school-age children that will enable them to achieve their optimum health and wellbeing.

Support Staff Key Responsibilities/Duties To work across the City as a member of the School Health team, participating in activities that address the health needs of school-aged children and young people, focusing specifically on children with complex needs in our special educational schools. To contribute to the identification of the Public Health needs of individuals and communities within the defined client group using evolving school profiles of health needs and working within specified agreed national and local targets. To plan, negotiate, and carry out initial and any subsequent ongoing child health assessments for a specific caseload, using relevant policies, procedures, and guidelines and making appropriate referrals to health and multi-agency partners as required. To assess, plan, and support children and young people regarding all continence issues, including nocturnal enuresis, toileting programs, and assessing if there is a requirement for containment products. Subsequently assessing and ordering appropriate products if deemed necessary. To develop, implement, and evaluate individual care plans for children with specific health needs. To adhere to Staffordshire Safeguarding Children Boards guidelines, to identify and report any case of suspected or confirmed abuse. To occasionally be the initial point of contact in identifying and reporting any case of suspected or confirmed child abuse.
The ability to manage child protection issues in a safe and consistent manner, which will include attendance at case conferences, reviews, and core group meetings. There is a need to remain objective while sharing sensitive information with the possibility that this may cause antagonism between parents and professionals. On average this may occur once a week.

Freedom to Act
To be responsible for making a professional judgment and acting on the correct interpretation of Fraser Guidelines, (a means of deeming whether a young person under the legal age of consent is competent to consent to treatment, without parental consent) in a variety of settings e.g. schools and clinics.
Initiates action to identify and minimize risk and ensure child protection safety, working in partnership with parents. This will involve working closely with Social Services and attendance at core groups and case conferences. Supervision will be provided by senior team members.
