This is a 12 month fixed term / secondment An excellent opportunity has arisen within Hambleton and Richmondshire Urgent Community Response (UCR) and Hospital at Home Virtual Ward Service. We are looking for a skilled and outstanding nurse to join our UCR and hospital at home service within the Community and Growing the Friarage Collaborative. If you possess innovation, dynamism, drive, and are passionate about patient-focused care, this could be the opportunity you are looking for. The community nurse in UCR and Hospital at Home Virtual Ward will support the admission avoidance agenda. Experience of working in a community setting, within an emergency assessment unit, or rehabilitation would be an advantage, and we encourage informal visits to the areas. The post holder will work as part of the multidisciplinary team within the Community UCR and Hospital at Home service to provide a comprehensive assessment and patient-centred care package, utilising support services and liaising with other agencies to ensure the needs of this client group are met. Must have access to a vehicle for work purposes to meet the requirements of the role. Main Duties of the Job
Be an active member of the multidisciplinary team by using professional judgement and assuming responsibility and accountability for assessing, implementing, and evaluating clinical care for patients to meet their needs in a safe caring environment that promotes dignity, privacy, and respect. Develop knowledge and skills within the specialty and assist in the management and organisation of nursing work in the ward/department within the NMC Code of Professional Conduct. Actively support the admission avoidance agenda, providing high-quality care to people in their own homes, recognising early symptoms of disease, providing appropriate responses to exacerbation and sub-acute illness, and maintaining a high-quality service, including palliative care and patients needing ongoing input within their own home. Work flexibly across 7 days a week, ensuring the 24-hour service is covered to meet the changing needs of the service, which may include working across the Middlesbrough, Redcar and Cleveland, and Hambleton and Richmondshire areas. Qualifications
Diploma or Degree in Adult Nursing. Registered as fit to practice by the Nursing and Midwifery Council (NMC). Evidence of Post Graduate clinical training/development. Knowledge & Skills
Effective written and verbal communication skills. Ability to effectively prioritise and coordinate own work. Ability to use initiative to make decisions within sphere of work and/or knowledge, recognising where assistance is required. Good information technology skills. Experience
Experience of multi-disciplinary working. Experience working within an acute or community setting. Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. Salary: £29,970 to £36,483 per annum (pro rata)
