The post holder will be an experienced health care professional competent in all aspects of care planning. Responsibilities include: Carrying out a wide range of clinical skills, including areas of specialist clinical expertise and advice within the team. Perform comprehensive assessments of patient nursing needs, plan, implement, and evaluate care according to changing health care needs. Work collaboratively within the multidisciplinary team/agencies to ensure that patient needs are met. Involve patients, relatives/carers in the delivery of care, providing reassurance and support. Prepare the clinical area for safe practice, ensuring cleanliness and readiness of materials and equipment. Recognize and respond appropriately to urgent and emergency situations. Maintain accurate patient records/documentation. Person Specification

Knowledge

Evidence of knowledge within the field. Detailed knowledge of professional accountability and NHS issues. Experience

Years of experience in a relevant field. Previous Acute Medicine Experience. Communication Skills

Ability to respond to the different needs of patients in various settings. Evidence of contribution to meetings/discussions outside of the team.
