Are you seeking to provide an opportunity for significant change in the lives of individuals impacted by substance use who are homeless?

If you aspire to work for a compassionate organisation that believes in people and possess the energy and determination to deliver healthcare interventions to the people most in need, this opportunity is for you.

We are recruiting a

Street Healthcare Outreach Nurse

in Ashton, Greater Manchester.

This is a new exciting project commissioned by The Ministry of Housing, Communities and Local Government, formally known as Department of Levelling Up Housing and Communities (DLUHC) and Centre for Homelessness Impact (CHI) to evaluate evidence of what works to end rough sleeping and reduce homelessness.

The CHI are looking to place 8 Street Healthcare Outreach Nurses in services that presently don't have this offer to evaluate the impact on the health of the local homeless population compared to areas that don't have this provision. As directed by the Lead Nurse, the post holder will be embedded within outreach services offering healthcare and clinical interventions as appropriate to the homeless population. Join our passionate team at the forefront of change, where every day brings the opportunity to touch lives and inspire lasting positive outcomes.

As a Street Healthcare Outreach Nurse, you'll not only provide high-quality clinical services but also embody the essence of empathy, understanding, and unwavering support. Your compassionate care will serve as a lifeline for individuals facing the challenges of homelessness, offering them not just medical treatment, but genuine compassion, kindness, and hope.

You will tailor care to the unique needs of each service user, guiding them toward a brighter, healthier future. Your experience, determination, and passion will drive the implementation of innovative clinical interventions, ensuring that every individual receives the care and support they deserve.

At Change Grow Live, above all else we believe in people. It's the one thing that acts as a foundation for all our work.

If you use our services: we understand what you're going through. We won't judge. We believe you can make positive changes in your life.

If you work or volunteer for us: we believe in your potential. We believe you can do brilliant, life-changing work.

Our mission is to help people change the direction of their lives, grow as individuals, and live life to its full potential.

The vision we're working towards is to develop, deliver and share a whole person approach that changes society.

We believe that having diverse people working as part of our team makes us the organisation that we are.

We actively encourage applications from people from all backgrounds to help us to provide the best possible experience for the people who use our services and to make Change Grow Live a great place to work.

The safety of vulnerable children, young people, and adults is our absolute priority. We will support you in your role to make sure that you are equipped to support the safety of people who use our services and those around them, to the highest standard possible.
