Full-time – 37.5 hours Salary Point 23 – 25/27 | £25,817 – £27,505/£29,345 We are looking for a Team Leader who has passion and vision for the delivery of excellent person-centred services, leading a team working to support 5 young men with learning disabilities and health needs. If you feel you have the relevant experience, commitment and drive to lead a skilled team, please get in touch. The post holder will work to deliver services ‘hands on’ as the leader of the team helping people to lead a fulfilling life within their home and community. As part of the Management Team, you will work to shape the service and to ensure high quality outcomes are achieved across Supported Living Services. You will deliver services on rota (that you will lead on) and the management task is to lead the team delivering high quality outcomes to enable people to live a fulfilled life. You will be responsible for ensuring that agreed outcomes from individual person-centred plans are met and to maintain excellent working relationships with family, key professionals and the wider community.
