Adult Social Work Services are recruiting for a Team Manager who will be an experienced social work practitioner and part of a new integrated multidisciplinary leadership team based at East Lothian Community Hospital. This team includes social workers, community care workers, physiotherapists, occupational therapists, and third sector representatives, focusing on preventing hospital admissions and facilitating timely discharges to improve therapeutic outcomes through rehabilitative care at home. You will oversee the main intake social work system in community adult social work services at John Muir House in Haddington, ensuring harmonisation between both systems for seamless progression of individuals through the care pathway into locality social work services. East Lothian Health and Social Care Partnership are adopting approaches that align with the new National Frailty standards and coordination of care via locality-based approaches with close links to the third sector. This role may involve some weekend working, with a requirement to work across 7 days initially over a 5-day working week, subject to review once the new pathway has been embedded. The post will be onsite, limiting the scope for home working. Interviews will take place on February 20th. Relevant Qualifications Degree in Social Work (or other recognised Social Work Qualification, e.g., CQSW, Dip SW, CCETSW) or equivalent level of qualification in Occupational Therapy.
