Assess, plan and review individual’s needs Promote independence and personalisation to ensure users can manage on their own Review ongoing service provision and attend service reviews with management Liaise and negotiate with local providers and support networks Provide information, advice and support to service users Skills & Experience
Social work qualification or similar relevant qualifications Good IT skills including Microsoft Office A high level of personal drive and is compassionate and empathetic Holds a full clean driving license Must have an Enhanced DBS Vacancy Summary
