We’re looking to recruit a Triage Support Administrator / Telecoach to support our Service User sign up process across our two South London contracts (South East and South West). The National Diabetes Prevention Programme (NDPP) is designed to support individuals to make long term behavioural change in order to help halt or reverse the progression of Type 2 Diabetes over a nine-month period. As a Health Tele Coach you will utilise your knowledge of health improvement coupled with your first-class communication skills. You will be encouraging clients to start their journey on the programme and ultimately achieve their health aspirations using motivational interviewing and behaviour change techniques. Triage Support Administrator / Telecoach

might include: Working with the Delivery Co-ordinator and Service Manager to ensure upcoming groups are fully booked with eligible service users. Conducting motivational interviewing over the phone, ensuring that all screening is carried out to the highest standards in accordance with NHS Code of Practice. Daily calls to lists of eligible service users across South London. Agreeing the most suitable pathway for potential service users (Face to Face, Digital or Tailored Remote). Delivering exceptional customer service whilst managing client referrals over the phone and electronically, and inputting all client referrals into our database. Working collaboratively with the rest of our National Diabetes Prevention Programme (NDPP) team as well as our partner organisations to create referral routes and meet targets. Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
