We have an exciting opportunity to join the Urgent and Emergency Care Division for 12 weeks as part of the system winter schemes. We are looking for 3 highly skilled clinicians to provide a Community Coordinator role based in the acute hospitals working across the Emergency departments, Frailty Units, and Assessment Areas. The individuals will be integral to support with avoiding admissions by diverting wherever it is safely appropriate, to community pathways and for those patients that are needing admission, to inform the Care Transfer Hub so that we can also support with timely discharge avoiding delays.

We have some incredible clinicians across SCHT who hold a wealth of community services knowledge that can support patients following an acute hospital attendance. Bringing this skill set and support to our acute hospitals during the busiest period will make a massive impact to improve outcomes for our local residents. The role will require you to work with the hospital clinical teams to promote and coordinate a smooth discharge/ step down pathway to appropriate services such as Virtual Wards, Urgent Response Teams, Therapy Teams, Respiratory Teams, Care Home MDT, Community Hospitals, District Nursing etc.

Main duties of the job

We are looking for applicants that share our passion for the specialist field of admission avoidance and complex discharge. The positions are an ideal opportunity to be part of shaping the service which is at the forefront of the NHS long-term plan agenda. They will be key to support the facilitation of discharge from the acute hospital setting to services that offer out of hospital care as an alternative to acute hospital admission. The Post holder will work within the Care Transfer Hub and clinical support and supervision will be aligned to the Virtual Ward clinical team.

The Post holder will be instrumental in the identification, assessment, and discharge of patients from the acute hospital setting urgent and emergency units, frailty and acute medicine and surgical units, agreeing the discharge pathway and services including home and bed-based reablement services, urgent community response (UCR) services including admission avoidance /Virtual Wards and community teams. They will lead on the discharge/transfer to community services supporting and advising hospital staff of the alternatives available and achieve a collaborative approach to safe and timely discharge from the departments.

Experience

Essential

Demonstrable experience of contact with patients both in an acute and community setting. Experience of working with multidisciplinary health and social care teams. Evidence of advanced clinical practice skills including advanced physical assessment, cannulation, phlebotomy, and interpretation of investigation results. Specialist and Expert knowledge in the field of admission avoidance and Hospital Discharge. Experience in leading staff and teams. Management of patients with long-term conditions, multi-morbidity, complex needs, including mental health. Demonstrable clinical supervision training or management experience or qualification.

Qualifications

Essential

Professional Registration (NMC, HCPC). Professional knowledge acquired through masters level qualification or significant equivalent experience or short courses in appropriate field to the equivalent level. Evidence of the achievement of leadership training or qualifications. Post registration qualifications in a relevant profession or business-related subject.

Knowledge and Skills

Essential

Well-developed communication (verbal and written) and interpersonal skills. IT skills in the use of Microsoft products (Word, Excel, Outlook, Publisher, Powerpoint etc). Ability to analyse administration systems, implement change as directed and evaluate effectiveness in order to promote quality services. Knowledge of Community services and resources. Well-developed communication (verbal and written) and interpersonal skills. Exercise judgments and initiative in highly sensitive areas when problems arise by taking appropriate action to resolve the problems or referring to the appropriate person. Able to prioritise and meet deadlines effectively. Ability to think and work creatively to resolve problems.
