Job summary We are looking for an experienced nurse to take up the Secondment post of Ward Manager to support the development of the following priorities and to support and drive a new model forward to deliver evidence-based practice to ensure we deliver the best patient outcomes: To provide outstanding step down and step-up rehabilitation To provide step-up care from the community, preventing admission to secondary care To provide palliative ward-based care for those at the end of their life To support the delivery of place-based care for people living in Mid and North Hampshire Alton Community Hospital works closely with GPs and local partners to ensure the population of Mid and North Hampshire have access to first-class rehabilitation and step-up care. You will work closely with the AHP manager to both support step-up care, preventing unnecessary acute admissions, and drive an outstanding rehabilitation model. You will lead the wider MDT, incorporating both internal and external partners, to provide the population of Mid and North Hampshire with seamless health care services with a home-first approach. Main duties of the job To provide clinical and managerial leadership to the day-to-day operation of the integrated clinical team and the ward environment. To support the day-to-day work of the clinical team, ensuring duties are discharged safely and effectively, and to provide expertise and guidance in complex and high-risk cases, in line with Trust and Local Authority policies and procedures. To promote a culture based on principles of recovery, choice, and personalised care, providing a role model to the rest of the team. To ensure the safe custody of all service users in accordance with their leave status as granted by the Responsible Clinician, including the supervision of patients within the unit, grounds, and community. To ensure all practitioners are working in line with the organisational policy and procedures for managing risk, including safeguarding adults and children. To ensure that lone-working arrangements are in place and followed at all times. To work across disciplines and agencies to ensure risk is managed (Police, Local Authority). To take all reasonable precautions to safeguard the welfare and safety of staff, service users, visitors, and the public, by implementing all policies related to health and safety, security, and risk, including infection control.
