Reablement is a free service available to anyone living in Bury who is aged 18 or over and needs support to get back on their feet after a recent stay in hospital or a crisis at home. It is a short-term service aimed at working with people to support them in managing everyday tasks independently in their own home, ultimately regaining their independence. You will be responsible for supporting customers with meal preparation, personal care, dressing, medication, and signposting them to services that meet their ongoing needs. You will contribute to the delivery, evaluation, and adjustments of goal plans to maximize independence outcomes and meet identified goals within agreed timelines. Proactively working in partnership with colleagues from health and other professional agencies is essential. You will maintain a safe and stimulating physical environment for customers, creating opportunities for them to be as independent as possible. Requirements include having your own car with business insurance for travel to customers' homes and the ability to work across all areas of the borough. A satisfactory DBS disclosure at the enhanced level is required, along with compliance with Care Quality Commission (CQC) regulations and the standards of the Choices for Living Well Handbook. Experience in supporting people in the community is necessary, and you should be prepared to complete the Care Certificate. Candidates should possess a 'Let's do it' approach to encourage and support customers towards independence and continued living in their own homes. You will use a work mobile phone and appropriate applications for contact during working hours and utilize electronic and IT equipment such as tablets and laptops in daily activities. Maintaining a high standard of personal appearance and cleanliness, including uniform maintenance, is expected. Your office base is at Killelea House, Brandlesholme Road, Bury, BL8 1JJ. Your line manager will be a Coordinator who reports to the Wellbeing Manager and then to the Registered Manager. Normal hours of work are 35 hours per week on a two-week rota: Week 1:
Monday, Tuesday, Saturday, Sunday 08:00 - 13:00, 16:30 - 21:30 Week 2:
