The ideal candidate will have experience in QOF, Chronic Disease Management - Diabetes, Asthma, COPD (preferably with Spirometry) Cervical Cytology. Immunisation, Vaccination and Family Planning skills are all essential. INR management and Systemone experience are desirable skills, however training can be provided for the right candidate. You will be able to communicate effectively with a wide range of healthcare professionals and be enthusiastic with a commitment to improving nursing practice through ongoing personal and professional development. We are a busy, supportive, friendly practice (14,000+ patients) located on the East Coast of Lincolnshire over two sites, in Mablethorpe and Sutton-on-Sea, and are part of a Primary Care Network. Marisco Medical Practice is rated Good by CQC and has a high QOF achievement. The clinical team consists of GPs, Advanced Nurse Practitioners, a Pharmacist, Practice Nurses and Health Care Assistants. Marisco is forward thinking and supports personal professional development whilst also being a training practice for universities, supporting Medical, Nursing and Nursing Associate students. JOB DESCRIPTION Special Requirements of the Post: 1.Registered General Nurse 2.Membership of a Professional Body 3.To work in accordance with the NMC Codeof Conduct 4.Ability to self-motivate, organise andprioritise workloads Job summary Job Title: Women Health Nurse Main Purpose of Post: Toassist medical personnel in the care of diagnostic centre patients to includetreatment, preventative care, screening and patient education Thepost holder is responsible for the delivery of Women Health nursing to the referred patients . The focus of the role is both the delivery ofevidence-based for the patients presenting Women health concerns, andthe provision of preventative health care to the practice population. As anautonomous practitioner the Practice Nurse is responsible for the caredelivered, demonstrating critical thinking and skills in clinicaldecision-making in the management of patients. They will work collaborativelywithin the general practice team to meet the needs of patients, support thedelivery of policy and procedures, and provide nurse support as required. Will actively promote women health with patients that are reffered to service. Thepost holder should work within the NMC code of conduct and ensure othernon-regulated team members work within their code of conduct. Key responsibilities Clinical practice Assess, plan, implement andevaluate treatment programmes that promote health and well-being Assess,plan, implement and evaluate individual treatment plans for patients with Women Health Concerns Proactivelyidentify, diagnose and manage treatment plans for patients as appropriate such as pessary changes. Work withother health care professionals to diagnose, monitor, manage, including non-drug-based treatment methods using amanagement plan, and in line with national and local policies. Work withpatients in order to support adherence to prescribed treatments Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions within the nursesspecialised role Supportpatients to adopt health promotion strategies that promote patients tolive healthily, and apply principles of self-care Support andmanage health needs of women presenting for ongoing family planning andcervical cytology consultations Advise,support and administer vaccinations where appropriate Confidentiality In thecourse of seeking treatment, patients entrust us with, or allow us togather, sensitive information in relation to their health and othermatters. business, and assuming responsibility inthe maintenance of general standards of cleanliness across the business inconsultation (where appropriate) with other sector managers Undertaking periodic infection control training(minimum annually) Communication Demonstrate sensitivecommunication styles to ensure patients are fully informed and consent totreatment Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating Anticipatebarriers to communication and take action to improve communication Estimateand maintain effective communication with individuals and groups withinthe work environment and with external stakeholders Act as anadvocate when representing patients and colleagues Recogniseand work within own competence and professional code of conduct asregulated by the NMC Produceaccurate, contemporaneous and complete
