- Facilitate safe and effective hospital discharges, working collaboratively with health professionals, patients, families, and community services.
- Apply a strengths-based approach using the Three Conversations Model to promote independence and reduce reliance on formal services.
- Support plans, ensuring they are outcome-focused and proportionate to the individual’s needs.
- Develop and implement person-centred care.
- Assess mental capacity and make best interest decisions in accordance with the Mental Capacity Act 2005.
Skills and requirements
- Registration: Must be registered with Social Work England.
- Experience: Substantial experience working with vulnerable adults, including those with complex needs.
- Full UK driving licence and access to a car is often required for community visits.
- Free DBS and compliance service.
