What the role involves

• Provide vital support to vulnerable adults as part of a fully integrated community crisis response team.
• Your responsibilities include undertaking statutory Care Assessments to identify eligible needs, submitting these to the panel for funding authorization, and conducting Mental Capa.
• While this role is primarily community-based, having experience in hospital discharge processes would be highly beneficial.

Skills and requirements

• To excel in this role, you must be registered with Social Work England, hold a degree-level qualification in Social Work, and have a minimum of two years of post-qualified experien.
