Where someone would start

Deliver exceptional social care to adults and their carers, promoting choice and control. Additionally, you will coordinate and deliver integrated care by working with health professionals, housing services, voluntary organizations, and other stakeholders. In this role, you will conduct strengths-based, person-centred assessments to identify the needs and strengths of adults within the community.

• To be successful in this role, you must have Social Work England registration and be eligible to work in the UK.
