The role:

To assist in coordinating the provision of seamless care, in conjunction with medical staff and to liaise with other members of the Multi-disciplinary Team to facilitate investigation, treatment, transfer or discharge of the patients to the heart failure ambulatory care pathway and the cardiology virtual ward as efficiently and effectively as possible.

Key responsibilities:

Clinical

  • Obtain and document full patient histories, often involving sensitive information.

  • Perform comprehensive physical and mental health examinations.

  • Screen for disease using clinical assessment, labs and imaging.

  • Make diagnostic decisions using clinical findings, ECGs, echo, angiography, ultrasound and x‑rays.

  • Use advanced clinical reasoning to form differential diagnoses and request investigations.

  • Perform appropriate invasive and non‑invasive procedures.

  • Interpret investigation results and collaborate with MDTs within cardiology protocols.

  • Act on results to confirm diagnoses and optimise treatment.

  • Develop evidence‑based treatment plans.

  • Prescribe medications and implement non‑pharmacological therapies.

  • Admit, discharge and refer patients as clinically required.

  • Provide patient and family support, education and guidance.

  • Review outpatients pre‑ and post‑procedure; obtain bloods; offer telephone advice.

  • Take systematic histories for elective admissions and ensure appropriate therapy.

  • Conduct full physical examinations and complete nursing admissions.

  • Screen all cardiology patients using clinical and investigation findings.

  • Interpret ECGs, perform manual defibrillation, order and interpret imaging/tests.

  • Deliver education on conditions, treatments, health promotion and secondary prevention.

  • Liaise with Cath Lab for elective admissions and theatre lists.

  • Manage MDT members and coordinate with hospital and community services.

  • Write Immediate Discharge Prescriptions to support timely discharge.

  • Assist with IV drug preparation and administration.

  • Provide advanced life support including CPR, IV drugs, venepuncture, cannulation and defibrillation.

  • Prepare and present cases for heart failure MDT

  • Lead and develop evidence‑based practice and professional standards.

  • Contribute to local policy and guideline development aligned with national strategies.

  • Work within NMC, NHS and HSE legal and ethical frameworks.

  • Promote patient empowerment through transparent, inclusive practice.

  • Participate in continuous performance monitoring and annual appraisal.

  • Maintain professional development through ongoing education and specialist involvement.

  • Identify patient and carer needs within the specialty.

  • Teach and mentor students and healthcare staff.

  • Promote research‑based practice.

  • Audit practice and implement improvements.

  • Evaluate role effectiveness against patient outcomes and service needs.

  • Participate in research/audit projects aligned with service priorities.

  • Ensure a high‑quality learning environment meeting NES standards.

  • Support staff undertaking research, audit or teaching.

  • Manage own workload according to job plan.

  • Oversee effective use of resources, supplies and equipment.

  • Contribute to strategic development of the nurse practitioner service.

  • Provide expert advice to support high‑quality patient care.

  • Promote a flexible, skilled nursing workforce.

  • Contribute to wider organisational initiatives.

    • Knowledge, training, qualifications and/or experience required to do the job:

  • ANPs are educated at Masters Level in advanced practice and are assessed as competent in this level of practice. As a clinical leader they have the freedom and authority to act and accept the responsibility and accountability for those actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems.
  • Registered Nurse on the NMC Register.
  • Be recorded as a Nurse Independent/Supplementary Prescriber (V300) with the NMC.
  • An appropriate Masters level qualification in Advanced Practice which has enabled competence in:
  • Clinical assessment
  • Clinical reasoning, judgement and diagnostic decision making
  • Anatomy and pathophysiology
  • Non-medical prescribing- V300
  • Leading, delivering and evaluating care
  • Practice learning/transferable work based learning
  • Significant comprehensive post registration experience within a healthcare setting some of which must be at a senior practitioner level.
  • A commitment to lifelong learning and demonstrates evidence of continuing professional development.
  • Can demonstrate experience in managing staff, leading a team, providing education and training and establishing a supportive, learning environment.
  • Successful track record in working in multi-disciplinary team settings and promoting collaborative approaches to delivering excellent patient care.
  • Advanced Life Support.
