Role overview

As well as identifying and signposting to other local services which will support their ongoing journey towards better mental and physical wellbeing. Create a non-clinical person centred support plan around the individual, guided by the individual’s needs.

Administrative work involved

Work with individuals and their carers around their mental health and social circumstances. Cultivate relationships with, and work with, other services around social wellbeing support for clients. Work with clients as the lead professional involved in their care, bridging to other services and opportunities and working closely with other organisations already involved.

Administrative skills and experience

• A minimum of 2 years community experience of mental health and wellbeing, and/or a qualification in appropriate subject , counselling, psychology, social work, mental health, Healt.
• Ability to work collaboratively within a multidisciplinary team (statutory and voluntary).
• An understanding of confidentiality, consent, information sharing, including GDPR.

Job details

• Interviews will take place on Thursday 9 July.
• Working with a caseload of clients with varied mental health challenges, this role involves helping them develop their potential through 1-1 advice.
• Support sessions, and supporting them to engage with an extensive range of activities and groups available to clients on the Community Connections Service.
• The Community Connections Service delivers its client groups and activities using both face to face and virtual platforms.
