CAMHS Liaison Lead Nurse/Practitioner focuses on provide 7/ day follow ups to work with young people and families in developing next steps in their care.

What the role involves

• Provide 7/ day follow ups to work with young people and families in developing next steps in their care.
• Safe discharge care plans and safety assessments being the priority.
• The team is 1/3 of South Derbyshire's Urgent Care Pathway that also comprises of a Crisis & Home Treatment Service and a Day Service.
• There is a 6 weekly afternoon where all services come together to reflect, learn and feedback, including external service introductions.
• The team operates from 8am - pm, 7 days per week.
• Training, as support, is offered regarding therapeutic interventions available within the offer.

Skills and requirements

• Registered Nurse/HCPC Registered Practitioner.
• Degree/ Diploma in Nursing/SW/OT Mentoring in Practice/Apple accreditation.
• Post graduate qualification in Child Mental Health.
• Qualification in a therapeutic modality.

Additional role context

• What sort of flexibility are you able to offer - We offer shift patterns 8am-5pm and 2pm-pm working across the community and office bases.
• We support each other across the team offering a split of Early and Late shifts and weekend cover.
