Care Coordinator (CPN / OT / Social worker)
Job description
This Care Coordinator role is focused on the team has regular clinical meetings to support with care planning and risk management as well as forums to discuss service development and quality improvement. It would suit someone who can bring warm, steady communication with learners to the role.
What the work leans on
To work collaboratively with service users focusing on a recovery approach and providing support and advice in relation to issues concerning them. Ensure a flexible approach in care provision with a focus on social inclusion opportunities and supporting service users to make informed choices.
Administrative work involved
- The team has regular clinical meetings to support with care planning and risk management as well as forums to discuss service development and quality improvement.
- Be based at Heavers Resource Centre which is located at 122 Selhurst Road, SE25 6LL.
- Meet service users regularly, within the timeframes required within their care plans, and follow through promptly on any actions agreed.
Administrative skills and experience
- Relevant professional qualification, i.e. registered mental health nurse, occupational therapist or social worker (A).
- Up to date registration with the appropriate professional body (NMC, HCPC, Social Work England) (A).
- Evidence of education and training relevant to the care of older people (A).
