Care coordinator
Care coordinator focuses on act as the first point of contact for our clients, their families, and healthcare professionals.
What the role involves
- Act as the first point of contact for our clients, their families, and healthcare professionals.
- Effectively manage rotas for your care teams, responding efficiently to any day-to-day changes.
- Monitoring care delivery to ensure it meets individual care plans and quality standards.
- Actively participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions.
- Supporting the out-of-hours, on-call service – with no requirement for hands-on care.
Skills and requirements
- A Full UK Driving Licence and access to your own vehicle, plus confidence to travel across the Northwest region (This will include motorway driving).
- Proven experience within a care coordinator role.
- Solid understanding of person-centred care and planning.
- Strong organisational skills, with the ability to manage multiple care packages and respond quickly to changing priorities.
