Care Coordinator focuses on performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.

What the role involves

• Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
• Monitor multidisciplinary collaboration of services to patients where appropriate.
• Assisting adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
• Must possess a degree in Health & Human Services or one of the following related Fields.

Confirmed role details

• Status: Full-time position (40 hours/week).
• 8am to 5pm EST, MF.
• This position requires both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+).
• Travel may include up to 90 minutes one way.
