Additional role context

• We are looking to recruit a motivated and compassionate Care Navigator to join our Transfer of Care Hub within Adult Social Care at Wirral Council.
• This is a key role supporting safe and timely discharges from hospital, working closely with health and social care professionals to ensure individuals receive the right support at.
• Help people transition from hospital settings back into the community, promoting independence and wellbeing.
