- This role specifically focuses on supporting young people aged 16 to 18 as they prepare to transition to independence or leaving care services.
- Managing a caseload of young people aged 16-18 within the Children in Care pathway.
- Maybe some court work involved.
- Care planning in line with legislation and local procedures.
- Undertake statutory visits, reviews.
- Work collaboratively with Personal Advisors, IROs, education providers, housing, health services, and other professionals to support the transition to independence.
Skills and requirements
- Qualified Social Worker with a Social Work England registration.
- Minimum of 2 years post-qualified experience within children's social care, preferably in a CIC or Leaving Care Team.
- Experience working specifically with older adolescents (16-18) and supporting their transition into adulthood.
- Enhanced DBS on the Update Service or willingness to apply for one.
