Skills and requirements

• Compass Associates is proud to be working with a well-established case management business, to recruit a new Case Manager to join their South East team.
• Involve carrying out INAs, conducting risk assessments and working with clients within their own homes and community settings.
• Work closely with clients and their families, whilst collaborating with MDT members, healthcare professionals and legal teams to deliver high-quality, person-centred support.
• Recognised Degree in Nursing, Social Work or Therapy (e.g. Speech & Language Therapy, Occupational Therapy or Physiotherapy).

Confirmed role details

• Home-based with travel across London/the South East.
• Full or Part Time (minimum 3 days/week).
• £48,000 - £50,000 (depending on experience).
• Regular team meetings for peer support, training and networking events.
