Clinical Pharmacist focuses on conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.

What the role involves

• Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
• Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
• Supporting safe prescribing through reconciliation, clinical screening, and dose adjustment.
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
• Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
• Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.

Skills and requirements

• A degree in Pharmacy MPharm, BSc or BPharm.
• Previous experience in a clinical hospital pharmacy role.
• Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
• Excellent communication and clinical decision-making skills.

Confirmed role details

• Up to £25.00 per hour (depending on level of experience, training and qualification).
• We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department.
• Provide cover for existing staff in times of annual leave or absence.
