Community Services Pharmacist

As a Community Services Pharmacist, the work centres on empowering patients to manage their medication safely, with support for developing confidence and capability in self-administration. It would suit someone who can bring delivering high quality personalised patient care in line with legislation, national and local guidance and specifications to the role.

How the role is set up

Optimise clinical outcomes, improve medicines safety and deliver best value from medicines within community services. To support the wider community services pharmacy team with the delivery of training to other members of the multidisciplinary team on prescribing and medicines related topics.
