CRHTT Nurse Practitioner focuses on care plans to patients and the families supporting them.

• Care plans to patients and the families supporting them.
• Commonly provide interventions and treatment for people presenting with co-morbid mental and physical health care needs, as well as complex biopsychosocial presentations.
• Our Home Treatment and Crisis service provision is a pivotal part of the acute care pathway for people with acute mental health difficulties.
• Your enthusiasm to provide the best quality of care could be just what we need to expand our team.
• Delivering the best care using the most up-to-date practise in supporting those with mental health illnesses.
• The post holder should be confident in assessing and managing mental health and social needs and be able to deliver a range of therapeutic techniques.

Skills and requirements

• Are you a newly qualified or experienced Band 5 RMN, looking for an exciting career within Crisis and Home Treatment team?
• Registered Mental Health Nurse (with NMC registration) and post qualification experience.
• Able to work autonomously and to deputise for senior staff in a variety of roles when required to do so.
• Support service users in challenging/crisis situations.
