Job description

The Health Coach position in Diabetes Prevention centres on the purpose of a health coach is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. Delivering evidence-based group 2-hour face to face sessions that focus on diet, nutrition, physical activity and behaviour change. · Engaging in ongoing training relevant to your role, such as Behaviour Change Level 1 and 2.

What you need

  • We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.

Practical information

  • Our Diabetes Prevention Programme consists of 13 sessions over a 9-month period, ordinarily we expect that you will be delivering multiple sessions per month.
  • The hours are a minimum of 12 hours per week, with the potential to take more.
  • Applications close: Wednesday 6th May, we reserve the right to close the application process early.
  • The group sessions are face to face, delivered in community venues across South London.
