This is a highly rewarding role where you will support her during her transition from a specialist neurological care and rehabilitation setting back into her family home, helping to ensure a safe, comfortable, and well-supported discharge process.

You will play a key role in delivering person-centred care, working closely with a dedicated care team to meet her clinical needs while promoting dignity, comfort, and overall wellbeing during this important stage of recovery.

At home, you will provide support with personal care, moving and handling, and a range of complex clinical interventions. You will also offer companionship and support her to engage in her favourite activities, providing meaningful interaction throughout both day and night shifts.

    Key Responsibilities

  • Supporting with personal care and daily routines
  • Assisting with medication administration
  • Supporting with moving & handling needs
  • Delivering complex clinical interventions as required
  • Providing companionship and emotional support
  • Promoting comfort, dignity, and wellbeing
  • Supporting both day and night care needs

    What We’re Looking For

  • Minimum 3 months’ care experience
  • Confident with medication, personal care, and moving & handling
  • Willingness to support with complex clinical care following training
  • Clear and effective communication skills
  • Willingness to travel for shadowing and training opportunities
  • A caring, patient, and adaptable approach

    What We Offer

  • Competitive pay with enhancements where applicable
  • Regular confirmed rotas and flexible scheduling
  • Reward & recognition programme
  • 24/7 support from a dedicated team
  • Ongoing training and development within a national provider
  • Our funded training (including clinical skills) is provided free of charge; however, attendance is unpaid.
