  • Recommend and arrange specialist equipment and home adaptations.
  • Supporting hospital discharge planning and rehabilitation pathways.
  • Provide advice and guidance to clients, families, and carers.
  • Liaise with Housing, NHS teams, voluntary organisations, architects, and external agencies.
  • Provide appropriate recommendations.

Skills and requirements

  • Qualified Occupational Therapist with relevant professional registration.
  • Experience working within community or adult social care settings.
  • Strong assessment and case management skills.
  • Excellent communication and partnership-working abilities.
