- Managing a varied caseload of hospital discharges and community referrals.
- Provide clinical oversight for reablement plans.
- Complete risk-assessed home visits.
- Supporting Reablement Officers delivering care programmes.
- Work collaboratively with a strong multidisciplinary team to achieve positive outcomes.
Skills and requirements
- HCPC registered Occupational Therapist.
- Minimum 2 years’ experience post-qualification (Band 6 level or equivalent).
- Proven experience in case work and physical rehabilitation in a busy environment.
- Ability to work flexibly: Monday–Friday, some remote work, and office-based two days/week.
