What the role involves

• Managing a varied caseload of hospital discharges and community referrals.
• Provide clinical oversight for reablement plans.
• Complete risk-assessed home visits.
• Supporting Reablement Officers delivering care programmes.
• Work collaboratively with a strong multidisciplinary team to achieve positive outcomes.

Skills and requirements

• HCPC registered Occupational Therapist.
• Minimum 2 years’ experience post-qualification (Band 6 level or equivalent).
• Proven experience in case work and physical rehabilitation in a busy environment.
• Ability to work flexibly: Monday–Friday, some remote work, and office-based two days/week.
