What the role involves

• Complete holistic functional assessments in homes and community settings.
• Recommend and arrange equipment and minor adaptations to support safety and independence.
• Apply strengths‑based, person‑centred approaches in all assessments.
• Review moving & handling plans, ensuring safe techniques and proportionate recommendations.
• Work closely with social workers, housing teams, health colleagues, families, and carers.
• Maintaining clear, accurate, and timely case records in line with service standards.

Skills and requirements

• Experience working in adult social care, community OT, reablement, or similar settings.
• Confident with equipment provision, risk assessments, and clinical decision‑making.
• Ability to manage an independent caseload in a busy environment.
• Free DBS and compliance service.
