- Complete holistic functional assessments in homes and community settings.

- Review moving & handling plans, ensuring safe techniques and proportionate recommendations.
- Work closely with social workers, housing teams, health colleagues, families, and carers.
- Maintaining clear, accurate, and timely case records in line with service standards.
Skills and requirements
- Experience working in adult social care, community OT, reablement, or similar settings.
- Confident with equipment provision, risk assessments, and clinical decision‑making.
- Ability to manage an independent caseload in a busy environment.
- Free DBS and compliance service.
