How the role works day to day

Provide advice, information, and signposting. Assess urgency of referrals and initiate enquiries. Transfer information to partner agencies in a timely manner.

• Rhondda Cynon Taf (Hybrid – Community & Remote).
• Full-time, 37 hours/week.
• Work model: Remote.

What needs to be in place

• Occupational Therapy Degree.
• Experience in adult social care and community-based work.
