This Practitioner Nurse opportunity is built around managing a caseload as keyworker/case manager, including clients with complex health and social care needs. It would suit someone who can bring monitoring effectiveness of clinical interventions, side effects and patient compliance to the role.

Where ownership sits

Thursday 0700-1500, Friday 0700-1430 Weekend 0800-1700 every second weekend on. Understanding of the application of the Mental Health (Care and Treatment) (Scotland)Actct 2003.

Care and support involved

Managing a caseload as keyworker/case manager, including clients with complex health and social care needs. Coordinating care under the Care Programme Approach. Maintaining effective relationships with service users, carers, relatives and internal/external professionals.

What helps in this care setting

Monitoring effectiveness of clinical interventions, side effects and patient compliance. A minimum of 27 days annual leave, increasing with length of service, plus public holidays. Membership of the NHS Pension Scheme, including life insurance benefits.
