- Undertake carers assessments and work with families.
- To provide information and advice to adults and their carers to help people stay healthy.
Technical requirements
- Act as Care Manager as required.
- To deliver the service in accordance with statutory responsibilities set out in the NHS and social care legislation and the eligibility criteria for services as agreed by the Counc.
