• To ensure that support plan reviews are carried out in a timely and person centred way evidencing need for continuing support.
• Undertake carers assessments and work with families.
• To provide information and advice to adults and their carers to help people stay healthy.

Technical requirements

• Act as Care Manager as required.
• To deliver the service in accordance with statutory responsibilities set out in the NHS and social care legislation and the eligibility criteria for services as agreed by the Counc.
