SEND Transition Advisor focuses on carrying out school engagement, personalised transition planning, and coordination.

What the role involves

• Carrying out school engagement, personalised transition planning, and coordination.
• Contributing to the EHCP consultation process.
• Engaging with students, families, and relevant stakeholders, and being a key point of contact for those with EHCPs prior to starting college.
• Having a caseload of EHCP students.
• Collaborate with the SEND and Quality Teams regarding emerging staff training needs.
• Line management of Specialist Learning Support Assistants.
