Senior Case Manager focuses on managing your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.

What the role involves

  • Managing your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.
  • Maintain Case Management plans with costed recommendations.
  • Identify and commission appropriate rehabilitation services for your clients ensuring service delivery is effective and managing any issues that arise.
  • Ensure all client documentation is accurate, up to date, and fully compliant with relevant regulations and standards.
  • Maintain solid relationships with stakeholders, business partners, regulatory authorities and existing and prospective referrers attending networking events as required.
  • Provide ongoing clinical and professional support to case managers, including regular client reviews and annual appraisals in discussion and agreement with the Clinical Lead.
