Senior Case Manager
Senior Case Manager focuses on managing your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.
What the role involves
- Managing your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families.
- Maintain Case Management plans with costed recommendations.
- Identify and commission appropriate rehabilitation services for your clients ensuring service delivery is effective and managing any issues that arise.
- Ensure all client documentation is accurate, up to date, and fully compliant with relevant regulations and standards.
- Maintain solid relationships with stakeholders, business partners, regulatory authorities and existing and prospective referrers attending networking events as required.
- Provide ongoing clinical and professional support to case managers, including regular client reviews and annual appraisals in discussion and agreement with the Clinical Lead.
