Candidate fit

• When it is part of an agreed care plan, to complete focussed work with individual young people using a range of interventions.
• Maintaining contact with young people’s families, carers and/or significant others offering support, information and where relevant to the care plan joint family and patient sessio.
• In conjunction with the responsible Social Services Department or integrated NHS assist in the assessment of suitable after care placement working across professional boundaries.
