What you would be doing

Coordinate a comprehensive holistic assessment of need and risk, establishing and reviewing an initial support plan agreed with the service user, carer(s), and service providers. Work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their fa.
