Support Coordinator focuses on we are recruiting a support coordinator for our floating support service in gateshead that provides outcome-focused care.

What the role involves

• We are recruiting a Support Coordinator for our Floating Support Service in Gateshead that provides outcome-focused care.
• Support to individuals who are at risk of homelessness and in need of housing related provision.
• Maintain tenancies in the local community.
• At our Floating Support Service, we promote inclusion, independence and wellbeing with the aim of enabling the people we support to become active and productive in their personal l.
• Provide direct support to service users in accordance with their holistic support plans.
• This role requires you to manage a team of recovery workers.

Skills and requirements

• Provide them with the knowledge of local service provision.
• Experience is essential for this role, as well as access to a car.
• We can only accept applications from candidates who are located in and eligible to work within the UK- t his post will not be open to Sponsorship under the UKVI scheme.

Confirmed role details

• Of working with Creative Support include a probationary bonus, pension contributions, 28 days Leave and company paid enhanced DBS.
