Working alongside care assistants to support residents with daily needs, ensuring residents remain as active and independent as possible. Case managing residents and liaising with families, healthcare professionals and other stakeholders to ensure that the best care plans are created and delivered. Working closely with the leadership team to develop services.

Practical details
