What the role involves

• Supervise the reablement social work team, providing guidance, oversight, and reflective supervision to ensure safe, strengths‑based, person‑centred practice.
• Manage workflow, including allocation of cases, monitoring performance, and ensuring timely assessments, reviews, and discharge planning in line with statutory duties.
• Work collaboratively with health and community partners, troubleshooting barriers, supporting effective multi‑disciplinary working, and ensuring individuals receive seamless, outco.

Skills and requirements

• Social Work England registration.
• Eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualified experience.
