- Supervise the reablement social work team, providing guidance, oversight, and reflective supervision to ensure safe, strengths‑based, person‑centred practice.
- Manage workflow, including allocation of cases, monitoring performance, and ensuring timely assessments, reviews, and discharge planning in line with statutory duties.
- Work collaboratively with health and community partners, troubleshooting barriers, supporting effective multi‑disciplinary working, and ensuring individuals receive seamless, outco.
Skills and requirements
- Social Work England registration.
- Eligible to work in the UK.
- Hold a full UK driving licence.
- Post-qualified experience.
